The surgical methods of a facelift

Various facelift operations require different methods:

Deep Plane Facelift is a gold-standard surgery of all the facelift procedures. It refers performing the dissection underneath the SMAS layer to widely undermine and release this layer, so that the sagging skin and also the muscle tissue will be tightened using stitches. Tightening the skin and muscle tissues gives a powerful lift effect, that also contours the skin of the face, beneath the muscles of the face. It also reduces wrinkles and removes excess fat. The risks of healing problems and irregularities are less and the results of a deep plane facelift can last from 15 – 20 years, unlike most superficial facelifts that only tighten the skin above the muscles.

Upper Facelift (cheeks or forehead lift) is achieved through an incision in the scalp of hair or on the forehead hairline and the subsequent tightening of the skin. The additional lifting of muscles and periosteum can be useful to get rid of even vertical forehead wrinkles, which can lead to temporary numbness. Besides forehead lift maybe the modeling of the cheek region will be required.

SMAS, Minilift, S-Lift, Mid Facelift (cheeks, neck or chin lift) This technique is performed with an S-cut and it is suitable for middle and lower Facelift (cheeks, neck and chin lift). It looks like S-shaped incision which begins with an incision in the hairline front of the ear, continues in a more or less straight line in front of the ear makes a lower arch that ends behind the back of the ear.

Neck lift: For the neck lift, the fat on the neck can first be aspirated. Then the sagging skin is tightened with the neck lift technique. The neck lift can be performed with a small incision under the chin or can be combined with a facelift. At the same time, the patient’s lower facial area can also be tightened.

With the Brow Lift operations the eyebrows are lifted up to where they should be. If necessary, it is possible to perform an eyelid correction with Brow Lift for a younger appearance.

There are 2 Brow Lift methods. Elevation method and endoscopic method:

1)  By the elevation method the eyebrows are lifted by a 0.5 cm incision in the scalp hair or on forehead hairline.

2)  By the endoscopic method the eyebrows are released through an incision in the scalp with endoscopic equipment from the bone and lifted to the desired position and fixed. This can be useful to get rid of even vertical forehead wrinkles.

After the Facelift Surgery

After the facelift, the incisions are still hidden and need rest in order to be able to heal. Caring for the scars is therefore very important. Under no circumstances should the patient remove plasters or bandages without consulting his doctor. The pressure on the cuts is important so that the later scars are as invisible as possible. The head bandage surrounds the forehead, cheeks and lower jaw. It protects the face and exerts the necessary pressure on the operated areas. With the help of the head bandage, the skin must reconnect with the underlying tissue. In addition, bruises must heal and the swelling must subside. For this it is important to move your head as little as possible in the first few days. Depending on the expected wound fluid, a drain remains in place for a few days. Drainage is important to the healing process and reduces the risk of complications.

The skin tension is normal for a few days after the procedure. The reason is not only the skin’s state of tension, but also the swellings on the face. Careful but regular cooling is important to reduce the swelling. A raised head posture also contributes to this. The feeling of tension subsides after a few days because the skin stretches a little and the swelling subsides. Numbness in some areas of the skin is also possible, but this usually subsides in a short time.

Physical exertion should be avoided in the first few weeks if possible. After about three weeks, there are usually no signs of the operation. Therefore it is possible to return to work during this period. Sport to a lesser extent is usually possible after three weeks, intensive sporting activity can be resumed after two to three months. Sunbathing and tanning beds should be avoided for six months as there is a risk of noticeable discoloration of the scar.
