Penis Enlargement Surgery

Penis enlargement, also called penile augmentation, refers to cosmetic procedures aimed at increasing penile length, girth, or both. Because these surgeries are strictly aesthetic—offering no functional or sexual benefits—urological societies often classify them as investigational and recommend thorough psychological evaluation before proceeding.

Main Procedures

Treatments are typically split into three main goals: increasing visible length, adding girth (thickness), or reducing surrounding tissue to success more visible penis.

– Lengthening: Surgeons cut the suspensory ligament that attaches the penis to the pubic bone. This allows more of the internal shaft to hang outside the body, typically adding 1 to 3 cm of length in the flaccid state. However, this procedure does not increase the length of an erect penis and can alter its stability during sex.

– Girth Enhancement with Fat transfer: To increase thickness, providers use either an autologous fat transfer, where fat is harvested via liposuction and injected into the shaft, or dermal fillers, like e.g. hyaluronic acid.

– Tissue Reduction: For men with “buried penis syndrome,” surgical removal of the pubic fat pad (suprapubic liposuction) is used to expose more of the natural length.

Risks and Considerations

While many patients report high satisfaction, these procedures come with significant potential complications. Fat transfers carry the risk of contour irregularities, unevenness, nodule formation, or the body reabsorbing the fat over time. More invasive surgeries like ligament release involve risks such as scarring, wound infection, and potential loss of sensation.
